Student-Athlete Medical Forms

All student-athletes must provide medical and insurance information each year. Please complete the returning student-athlete packet if you participated in a varsity sport or cheer in 2010-2011. If you are a new student or did not compete last year, complete and return the new student-athlete packet. All documents must be submitted by August 1st, 2011. You may open the file as a pdf in Adobe, but be aware that you may not save any changes. Please print the form once complete and ready to mail.

The medical forms for 2012-2013 will be available here after June 1, 2012. Please access them after that date, complete, and return all forms by August 1, 2012.
